Locate L5, which corresponds to the transverse processes immediately cranial to the tuber coxae 2.Locate the transverse processes of L1 and L2 by counting cranially from L5. L1 has shortened transverse processes which are buried in fat and so are difficult to palpate. Its location needs to be estimated by gauging the distance between L2 and L3 and extrapolating this to L1 and L2. 3. Place a bleb of local anaesthetic over the middle of the transverse processes of L1 and L2 to reduce pain. 4. To anaesthetise T13, introduce a 19 guage , 5cm needle through the bleb over the transverse process of L1. Advance the needle ventrally until the bone of the transverse process can be felt. 10ml of fluid is injected into the site. 5. Walk the needle off the cranial edge of L1 and advance it ventrally, if the needle enters to far, it enters the abdominal cavity where there is negative pressure and this can be observed by placing a small amount of fluid in the needle. 10ml of Lidocaine is injected into the site 6.
